What is Home Health Care?

Ivy Creek Home Health of Elmore is an extension of the care you receive from a hospital and your physician in the comfort and privacy of your home. A plan of care is developed with your physician and closely supervised by our competent and caring staff, insuring continuity of care.

Our Home Health Services

  • Development of individualized physician directed plan of care by a Registered Nurse.
  • Skilled nursing care provided by Registered Nurses and/or Licensed Practical Nurses.
  • Personal Care and hygiene services provided by Certified Home Health Aides.
  • Strengthening and restorative physical therapy supervised and provided by a Registered Physical Therapist and Licensed Assistant.
  • Additional services include Speech and Occupational Therapy provided by a Licensed Therapists.
  • Other services include arrangements for special equipment such as hospital beds, wheelchairs, home oxygen, etc.

Who Qualifies for Home Health

 To qualify for Ivy Creek Home Health services, a person should be essentially homebound. However, the patient does not have to be totally confined to the home to qualify. A person is considered homebound if it would require “considerable and taxing effort” for the person to leave his or her home.

Additionally, the patient should be under the care of a physician who will approve a written plan of treatment for the patient.

The last qualification is that the patient needs intermittent care. The term “intermittent care” means care that is only needed a few hours a week.

How to Obtain Services

A physician’s referral is required to initiate home health services. Your questions can be answered by the following people who will also help to make the necessary arrangement. Your Physician and his staff, your hospital discharge planner, or simply call us at (334)-567-4311.

Our Health Care Professionals

Ivy Creek Home Health is staffed by trained professionals. All are experienced care givers with hospital, nursing home, or other professional experience. All staff members participate in continuing educations programs on the latest medical skills and techniques.

Who Pays for Home Health

  • MEDICARE, if eligible and qualified.
  • MEDICAID, if eligible and qualified.
  • Private Insurance, when home health care is covered.
  • Private Pay, with personal financial arrangements.
